To be scheduled, new clients must complete our intake forms and send the patient’s medical records. Please download the forms below and email your completed forms and medical records to museholisticvet@gmail.com.

If you need help requesting records, please email us and provide the following:

• Your Full Name
• Your Pet’s Name
• Your Vet’s Name and Location

• Your appointment will only be confirmed after Dr. Barrett has reviewed your case. Kindly allow 1-3 business days for Dr. Barrett to give the confirmation via email.
